Independent review of the Dioxin incident in Northern Ireland

A recall of all Irish pork products produced from pigs slaughtered in Ireland was ordered by the Food Safety Authority of Ireland on Saturday 6 December 2008. This resulted in a decision by the Executive, in a meeting on Thursday 11 December 2008, to commission a review of the handling and communications of the incident.  
In accordance with the principles of executive accountability for safety of the public and sound administration, Kenneth J MacKenzie CB was asked to conduct an independent Review that would guard against any repetition of such an incident by establishing what had gone wrong and identifying lessons learned.
